| Do I have a copay?
If you have a copay, the copay amount is included on the front of most insurance cards. If I have a copay, when is payment due? Payment is due at the time of service. Payments in the form of Cash, Check, Visa and Mastercard are accepted. Do I need to get a referral for my appointment? Most Health insurance cards will say whether or not a referral is needed. It is the patient's responsibility to have a referral, if one is required. What if I need to cancel my appointment? We do ask that appointments be cancelled at least 24 hours prior to the scheduled time. Patients who do not show or call may be charged a fee of $25.00 for doctor's visit and/or $50.00 for missed testing. Do I need to bring anything with me to my appointment? Please bring your insurance card/s, driver's license or state issued ID, any paperwork mailed to you, referral (if needed) and radiology reports and/or discs. I am scheduled for a PFT, is there anything I should do prior to my appointment? Do not smoke before testing. Avoid caffeine. Do not use your fast-acting rescue inhaler 4 hours prior to testing. Do not wear perfume, cologne or lipstick. |
